Job scope:  

Care Coordinator scope:

LVN performs credentialing and privileging activities for the organization as required by HRSA, CMS, and other state or accrediting bodies. LVN monitors and maintains health plan enrollment/participation for patients and providers. LVN performs duties related to the coordination of clinical care based on needs of patients and available resources.

Clinical scope:

LVN performs routine administrative and clinical tasks to keep the offices of physicians and other health practitioners running smoothly. The duties of LVN varies from setting to setting, depending on the size, location, and type. In addition to administrative duties, LVNs perform clinical duties in accordance to state law, competency, licensure, and education.  The clinical duties will include but not limited to taking medical histories and recording vital signs, explaining treatment procedures to patients, preparing patients for examination, and assisting the physician during examinations.

Education:  High school graduate or equivalent.  Graduate of an accredited Licensed Vocational Nursing program. Current licensure by the Texas State Board of Nursing. 

Position requirements: Able to read and write consistent with job requirements; cognitive skills as related to the position.  Proficient interpersonal relations and communicative skills. Understanding of medical terminology.

Physical requirements that are bona fide occupational qualifications essential to the performance of the job:

Ability and strength to sit, bend, and reach during an 8 to 10 hour day.  Able to lift a minimum of 50 pounds.  Sensory acuity sufficient to evaluate, treats, and monitors patient needs and maintains accurate records.  Fine motor skills sufficient to legibly document and communicate.  Routine 8 to 10 hour shifts.
