Mental Health Case Manager Trainee

Position Overview: We are seeking a dedicated and compassionate case managers to join our FIRST (Foundations, Independence, Recovery, Support, & Tools) Story Changer Program. This position begins with a comprehensive training program covering the foundations of case management, individual engagement, community resource navigation, and documentation best practices. The successful candidates will be expected to demonstrate competency in training topics and essential case management skills to continue employment in the program and be considered for full-time case manager positions. This role is ideal for individuals who are motivated to learn, passionate about helping others, and ready to grow in a meaningful career. Applicants are advised that being hired into the FIRST program does not guarantee continued employment with Community Healthcore as a case manager or otherwise on a full-time basis, as employment with Community Healthcore is at-will. Being hired into the FIRST program does not constitute an employment agreement or contract that would alter that relationship.

RESPONSIBILITIES:

  • Participate fully in the initial case management training program
  • Build knowledge of community resources, individual advocacy, and care coordination
  • Work collaboratively with supervisors, peers, and community partners
  • Build knowledge of individual engagement, assessments, trauma-informed care, person-centered planning, case management, and service provision
  • Negotiate and facilitate the array of services and supports needed to address the individual's identified goals and desired outcomes.
  • Demonstrate critical thinking and problem solving skills
  • Provide behavioral intervention services and document service provision

QUALIFICATIONS:

EDUCATION AND EXPERIENCE:

  • Bachelor's degree or master's degree in a Human Services field, including but not limited to: psychology, sociology, social work, rehabilitation, counseling, human growth and development, gerontology, special education, educational psychology, early childhood education, early childhood intervention.

CERTIFICATIONS, LICENSES, REGISTRATIONS:

  • Valid Driver's License
