QMHP Crisis Screener

Job Scope: Delivery of Service Coordination Services as primary function. However this position will also include regular crisis screenings and brief crisis intervention services to consumers. The QMHP screener will provide Crisis Screenings, Crisis Interventions, Hospital aftercare follow-ups, Service Coordination, ANSA/CANS completions, client transportation, and other ancillary services as indicated. Adjunctive services include coordination with other providers/agencies, client staffing, and serving as a resource for other CHC staff. This position is a hybrid position, in which services can be provided in person or via telehealth dependent on the needs of the individual in crisis. The QMHP will use professional and technical skills as described in Core Competency Inventory. This professional position helps individuals to lead lives of dignity and independence by assisting individuals in crisis to maintain community tenure, thus avoiding psychiatric hospitalizations. This position also helps individuals transition back to the community from a psychiatric hospitalization. This position requires the ability to effectively communicate with a very diverse population. Also, as part of this job there will be weekend work and mandatory deep nights as needed.

Education/Licensure:

Bachelor's degree in a Human Service field that meets/exceeds the criteria of QMHP

Current valid driver's license in state of residence.

Experience/Knowledge:

Requires a basic understanding of mental illness, assessment processes, and how to access professional/community resources. Must have excellent written and verbal communication skills.

Physical Requirements:

Mobile and able to bend and reach during 8-12 hour day

Must be able to adequately complete SAMA, First Aid, and CPR training

Work endurance sufficient to work an 8-12 hour day
