Job scope: Duties are carried out in accordance with nursing licensure standards and established policies and procedures of Community Healthcore; functions as the primary accountable person for consumer medical services in the IDD programs; coordinates the delivery of Medicaid waiver and non-waiver services in compliance with the IDD provider principles for evidentiary certification; assists the QIDP/Case Manager in developing individualized services for each consumer based on the needs, preferences, strengths and assessments; advocates for and ensures consumer/family/legal guardian satisfaction in services; functions as a team member with the QIDP/Case Manager, Home Manager, Program Manager, Program Director, Director of Nursing and Direct Care staff to deliver services according to the plan of care (Person Directed Plan/Individual Program Plan); monitors the delivery of medical services.  Provides RN delegation for non-medical staff.

Education/licensure: Graduate from an accredited school of nursing; Licensed to practice as a Registered Nurse in the State of Texas; Current CPR certification; Must have a driving record insurable by the CHC's insurance administrator.

Experience/Knowledge: Demonstrates knowledge of standards of nursing practice for Registered Nurses.  Knowledge of the nursing peer review process; working knowledge of licensing standards for services and programs of the center; three years related experience preferred.

Physical requirements that are bona fide occupational qualifications essential to the performance of the job: Position requires various activities involving visual, auditory, motor skills to communicate; attend meetings/trainings, carrying charts and assisting with individuals (up to 50 lbs.) during an 8-hour day; monitor individual needs and to maintain accurate records; fine-motor skills for legible and accurate writing of reports, charting, scheduling, and daily correspondence; endurance to work an average 8-hour day - up to 12 hours may be required on isolated occasions.
