Job scope:The Community Case Manager will be assigned to the East Texas Veterans Resource Center within the Supportive Services to Veterans/Families (SSVF) program. Duties will include completing a comprehensive Intake and Assessment to determine eligibility; conducting Coordinated Entry assessments for veterans experiencing a housing crisis; effectively manage their caseload to ensure program participants receiving services in a timely manner; being knowledgeable of community resources, especially those resources related to rapid re-housing, homeless prevention, veteran mental health, and veteran support services; participate in the Local Homeless Coalitions within the counties of service; develop relationships with landlords within the counties of service; conduct program outreach within the counties of service; and complete funder-required reporting. The Community Case Manager will also attend required trainings, including Veteran/Family Peer trainings.
Education/licensure:Current valid driver's license in state of residence; Bachelor's Degree in a Human Services field.
Experience/Knowledge:Preference given to a Veteran or family member with first degree relationship; preference given to applicant with knowledge of the VA system and veteran community resources.
Physical requirements that are bona fide occupational qualifications essential to the performance of the job:Position has requirements to perform lifesaving activities such as CPR and SAMA techniques.
