QMHP - Care Coordinator

Job scope: This position is part of the Care Coordination team and assist individuals to lead lives of dignity and independence through the completion of needs assessment and the provision of support activities in accessing resources including housing, employment, basic needs, primary care, and linkage to natural supports. The QMHP will continue to assess and follow up on individual's needs, and level of satisfaction. This position will encourage the use of home and community resources, appropriate to individual's preference and needs, to reduce the risk of crisis and allow the individual to remain in the least restrictive environment. Insures documentation is completed within required guidelines and meets or exceeds DSHS standards. Is knowledgeable of community resources and effectively engages individual. Insures individual and other stakeholder participation (as is appropriate) in all aspects of Care Coordination support activities. Effectively communicates and interfaces with allied service providers.

Education/licensure: Bachelor's degree in a Human Service field that meets the criteria of QMHP

Current valid driver's license in state of residence

Experience/Knowledge:

Requires advanced organizational skills and ability to manage multiple tasks.

Functions as a team member and actively participates in continuous quality improvement activities.

Must demonstrate understanding of cultural sensitive, trauma informed and person centered principles and practices.

Requires an understanding of mental illness, behavioral health issues, assessment processes and how to access professional/community resources. Must have excellent written and verbal communication skills.

Physical requirements that are bona fide occupational qualifications essential to the performance of the job:

Mobile and able to bend and reach during 8-12 hour day

Must be able to adequately complete SAMA, First Aid and CPR training

Work endurance sufficient to work an 8-12 hour day
