We provide access to holistic care to the individuals we serve including primary healthcare, mental health medication management, skills training, psychosocial rehabilitation, counseling, and connection to other services need for recovery. Requirements: Bachelor's or master's degree in counseling, psychology, social work, sociology, or related fields.

Minimum Hiring Salary Annualized: $41,600 (Bachelor level) / $43,805(Master level) / $45,011 (Therapist Associate)

Mental Health Case Manager - Adult Services

An individual in this position provides skills training services, casework services, screenings, plans of care, and coordination for aftercare to our adult clients. Our service area for positions in this posting covers the following counties: Gregg, Harrison, Marion, Rusk, Upshur, and Panola.

IST: Intensive Service Team

This position is designed to be part of the  Assertive Community Treatment (ACT) team which is a multidisciplinary team providing intensive, community based outpatient services for  participants with severe and persistent mental illness (SPMI) and co-occurring disorders. 

As a case manager for the ACT team, you will walk alongside program participants and be a key player in supporting their access to community resources, access to housing supports, and other entitlements. In doing so, your role will provide clients the opportunity to build their skills, develop healthy relationships with the client and community agencies, family, and other natural supports and facilitate progress towards overall wellness and healthy functioning.

Your skills in quick problem solving, time management & communication, crisis support, and advocacy will be instrumental in your success within this role as you partner with your fellow team members, internal PCBH partner programs and other community agencies to facilitate clients' success in recovery and achievement of their identified treatment goals. Early Psychosis and Intervention Program

The First Episode Psychosis program is a holistic treatment approach that includes: • Psychotherapy, especially cognitive behavioral therapy. • Family support and involvement. • Support from peers in recovery from mental illness. • Antipsychotic medications prescribed by a psychiatrist. • Support for education and employment goals.  Serving individuals age 15–30 years old with a psychotic disorder.
