Outreach Coordinator | Texarkana

Job scope:  Position is the focal person providing person centered Information and Referral (I&R) to callers and walk-ins for the 14 county service area.  I&R services includes (1) excellent customer service, (2) linkage to appropriate long term services & supports and other local resources, (3) concurrent data entry into tracking system, (4) completion of the Long Term Services and Supports (LTSS) screen as required, (5) maintain / update regional resource database, (6) assist persons with Your Texas Benefits portal through the Community Partners Program and (7) create reports for Texas Health and Human Services Commission and Community Connections Building performance metrics. 

Position is the lead for the Local Contact Agency (LCA) role to (1) provide Options Counseling to non-Medicaid referrals from Nursing Facilities, (2) serve as a resource for hospital and nursing facility discharge planners and PASRR referrals, (3) enhance existing community resources to support transition from Nursing Facilities to the community, (4) develop and provide community training regarding the role of the Local Contact Agency and (5) complete all reporting to document LCA efforts to Texas Health and Human Services Commission.   

Position will works with other ADRC staff and community partners to build awareness of ADRC services in the 14 county service area through events that may occur on the weekends and evenings.  Assist with other duties as assigned.

Education/licensure:  A Bachelor's Degree in a Human Services field is preferred.  However required is an Associate's Degree in a Human Service field or High School diploma with two years' experience serving with good phone skills.  It is also preferred to have strong verbal and written communication skills.  Current Texas driver's license.  Within year two of employment will be certified as a Level 1 Benefits Counselor Texas Health and Human Services Commission and completed training related to Long Term Services and Support screening, Motivational Interviewing and Person Centered Thinking.  Within three years will be Alliance of Information and Referral Systems (AIRS) Certified.

Experience/Knowledge:  Working knowledge of office organization and management, business English, computer word processing, power point, spreadsheet and data base software.  Good listening and communication skills.  Ability to establish and maintain effective work relationships with other employees, community partners, and the public.  Prefer experience in the human service field.

Physical requirements that are bona fide occupational qualifications essential to the performance of the job:

Operating office equipment, use a computer; operate motor vehicle ability, and the ability to sit for more than 2 hours.  Hearing, ability to read, ability to count, see, write, communicate verbally, and the ability to move about in most environments.
