Job scope: Under the supervision of the Program Manager, Staff will provide individualized and group training/ activities based on the Independent Life Skills (ILS) Assessment and what the person(s) is interested in. The assessment will identify what needs the individual has, i.e., life skills, independence, community integration. Adheres to individualized plan of care, personal interests, and objectives. Staff will provide ILS services at the location of the individual (Nursing facilities) or offsite. Staff will participate in group outing, and nursing facility activities. Staff will research and develop community integration services, resources, and other activities that will meet the needs of the individuals.Documentation of client progress is reported accurately in progress notes and daily group service log. Staff is responsible for entering data into credible daily. Other duties as assigned.
Education/licensure:
High school diploma or GED
Current valid driver's license in state of residence.
Experience/Knowledge:
Ability to communicate effectively both orally and in writing. Able to work cooperatively with consumers and family to establish a positive relationship.
Physical requirements that are bona fide occupational qualifications essential to the performance of the job:
Able to lift up to 50 pounds. Able to transport consumers between location and other training activities.
Minimum Hiring Salary: $16.00 per hour
