Peer Provider-Care Coordination | Longview Area

Job scope:

Under the direct supervision of LPHA, the Care Coordination Peer Support Specialist draws from their own personal experiences and comfort level to provide individualized support activities according to individuals' recovery goals. The Peer Support Specialist will be able to share a hopeful message and exercise choice to assist individuals in being able to live, work and participate in their home and community. Peer Support Specialist will utilize different pathways to recovery to investigate, select, and use needed and desired resources including housing, employment, and access to resources to meet basic needs, primary care, and linkage to natural supports. Services shall typically be provided in individuals' natural environment. Requires the ability to establish good personal boundaries, and be able to effectively communicate considering the uniqueness and complexities of individuals and their families.

Education/licensure:

Current valid driver's license in state of residence.

Graduation from High School or Equivalency

Experience/Knowledge:

Prior experience as a recipient of public mental health treatment or rehabilitation services.

Ability to effectively communicate with a wide range of individuals.

Demonstrates ability to construct grammatically correct descriptive paragraphs.

Ability and willingness to follow job related instructions and accept supervision.

Displays ability to maintain appropriate personal boundaries.

Minimum Hiring Hourly Salary: $16.00
